It's our joy at Plus One Pelvic Health to be able to give something back to our community. We know not everyone can afford the full cost of one-on-one pelvic physical and occupational therapy, so we are sponsoring another round of in-person sessions to someone special! 

You can nominate yourself or someone you love by using this form. Please include any relevant information in your nomination as well as the reason(s) they should be selected.

 ***Sponsorship is currently for 50% of appointment cost***

Nomination criteria:

1. Woman ages 16+

2. Able to come to our office in Franklin for all appointments (or virtual appointments as needed)

3. Able to commit to therapy and plan of care determined by the patient and therapist (usually weekly appointments)

4. Have a pelvic health need, such as incontinence, pregnancy, postpartum, diastasis, dyspareunia, pelvic pain, etc. and/or a need for somatic experiencing trauma therapy

5. Able to pay for half of treatment costs
